Holy Trinity Catholic Church’s Family Faith Formation program provides a clear path for children preparing to receive their Confirmation.

Prayer Review
Prayer Review will take place quarterly with your Group Leaders. A list of all the prayers your child needs to recite each quarter is below.
Saint of the Week!
Each week, complete a little lesson on a saint so you can learn more about them and their lives. Maybe you will be called to choose one of them as your Confirmation saint! To help keep on track, the forms will close on the dates indicated so be sure to complete them on time. All the saint forms for each quarter will be open until their close date, so feel free to work ahead!
You will receive an email confirmation that we have received and recorded your saint form. Please wait until at least one week past the due date for that confirmation.
If you miss a week, we will not be opening the saint forms again, but the candidates can make up the saint form by using the biography link and following the steps below. These make-up assignments MUST be turned in by the end of each quarter to receive credit.
How to Submit the Saint Form Make-up:
- Under each saint’s form link, there is a link to the biography of that saint. Have your child read the biographies for the saint forms they are missing.
- After your child reads each biography, he/she needs to write FIVE meaningful facts that were learned about each saint (NOT just the year they died or what they are the patron saint of, however, ONE of these types of facts is acceptable per saint).
- Every saint your child researches should be written about separately in one document. Please only send ONE typed document with ALL the saints needed to complete for the quarter included. The write-ups for each individual saint can be written as a list of facts or a paragraph. Please have your child title each saint write-up with the week number and saint name (ex: Week 6: Saint Benedict). Make sure your child puts his/her name at the top of the document!
- If your child turns in this make-up incomplete (not enough facts or incorrect information) he/she will not receive credit for completion (obviously) so please review the document with your child before you send it!
- This make-up assignment should be emailed to [email protected]. Please use this subject so it is not missed: Saint Form Make up- Child’s First and Last Name (ex: Saint Form Make up- Sally Jones)

Building Your Catholic Library
Having little ones is a great time to start building a library of wonderful Catholic books and resources. Below are some recommendations. They make great presents! (COMING SOON)
The library of teaching materials should include:
- Kids’ version of Catholic Bible
- Several children’s catechisms
- Kids’ missal and prayerbook
- Coloring and activity books
- Children’s books on lives of Saints

Prayer Review
Prayer Review will take place quarterly with your Group Leaders. A list of all the prayers your child needs to recite each quarter is below.
Holy Hour
Time spent in quiet prayer in front of the Blessed Sacrament in Eucharistic Adoration is transformational. As St. Theresa of Calcutta taught, “The time you spend with Jesus in the Blessed Sacrament is the best time that you will spend on earth. Each moment that you spend with Jesus will deepen your union with Him and make your soul everlastingly more glorious and beautiful in heaven, and will help bring about an everlasting peace on earth.”
Each candidate will attend a holy hour with a parent or sponsor and use the guided meditation provided by the Faith Formation Office. The candidate and parent or sponsor will go to our parish adoration chapel to complete this requirement. We will have a list of the candidates on a clipboard in the adoration chapel. When you enter the chapel, please find your name and sign in.
You will need to use the programs linked below (one for the candidate and one for the parent or sponsor). They can be found on the clipboard. Please return your program at the end for others to use. You can also print your own if you enjoy journaling while praying.

**If your child was baptized at Holy Trinity, you DO NOT need to submit their baptismal form. We have that record already.**
Please turn these important forms into the faith formation office or your group leaders if you are part of the program. If you are doing the Parent to Child Program, HTH, or Catholic School, please use your family folder in the ministry closet. Please do not email us your paperwork!

Catholic School HTH
Requirements
Since the diocese requires two consecutive years of documented faith formation prior to a sacrament, please send a brief email after registering stating that your child successfully completed his/her first year of sacramental prep and completed your program requirements in the program. It will be printed and filed with your child’s sacramental records. The email should be sent to [email protected]
1st Year First Confirmation
There are no extra parish requirements. We will reach out to you at the end of the year to ask for a letter stating that your child successfully completed his/her first year of sacramental preparation.
2nd Year Confirmation

Confirmation Data, Sponsor Eligibility, and Letter of Intent
They should be turned into the Family Faith Formation office using the Black Plastic Tray labeled 2026 Confirmation Paperwork provided for you in the Ministry Closet. When you walk into the closet, there is a table to the left. The black tray will be on top of that table. DO NOT email us these important forms!

Do my Catholic School or HTH children need to attend the weekly parish Family Faith Formation classes?
Catholic school families are exempt from participating in the parish Family Faith Formation classes as your children are receiving faith formation classes 5 days a week in their parochial schools.
HTH members are exempt from participating in the parish Family Faith Formation classes as you are already implementing faith formation every day and all day as a Catholic homeschooler.
Do my Catholic School or HTH children need to register with the parish Faith Formation office every year?
YES! Please register your children (1st-8th) with the parish Faith Formation office every year, even though you do not need to attend the classes.
Why? Because it helps our pastor show the diocese the demographics of his parish. If our parish looks like it only has 400 children in grades 1st-8th, we do not show a correct picture of our parish to the diocese. Currently, on paper, it looks like we have many families at HT, but very few children.
How do I register my Catholic School and HTH children with the Faith Formation office?
All parish families (including Catholic School/HTH) MUST register any child who will be receiving a sacrament in 2025 and 2026 at Holy Trinity. Registration will run June 1-July 31.
The cost is $40 per child for children receiving a sacrament in 2025 (2nd year First Holy Communion and 2nd Year Confirmation) to cover retreat and other material expenses. There is no fee for the children who are receiving a sacrament in 2026 (1st Year First Holy Communion and 1st Year Confirmation).
Registration of your non-sacramental children who are in 1st-8th grade costs nothing, so please also include them on the registration form.
What do my Catholic School and HTH children in a sacramental year need to do?

OCIC/OCIT Preparation
If you have a child who has been baptized and is age 8 or over (in the 3rd grade at least) and has not yet started any faith formation or if your child has had a 2 year gap in verifiable faith formation, please read the information below about registering for OCIC/OCIT sacramental preparation classes.
OCIC/OCIT Information:
- Classes are Monday evenings 7:00-8:30pm.
- A parent is required to attend class with your child(ren).
- Classes are $100 per child.
- These classes are the FIRST year of sacramental preparation for the children.
Your child will receive FHC after 2 consecutive years of formation. - At the end of the school year, in the spring of 2026, there will be a required family interview for all candidates seeking CONFIRMATION. This family interview will be an assessment of readiness to move forward with the next year of formation.
- This is no longer a TWO YEAR program for CONFIRMATION. Completion of 2 years of classes does not guarantee reception of CONFIRMATION. After 2 years of formation, reception of CONFIRMATION is dependent upon active participation in classes, regular attendance, timely completion of all assignments, and a successful interview with the clergy at the end of year one and year two.
- You may not miss more than 4 classes the entire school year. If a class is missed, the parent is responsible for getting the make-up lesson and presenting the content to your child.
